Eligibility
LONG TERM CARE (LTC)
- Medicaid recipients who are 65 years of age or older.
- 21 years of age or older with a disability.
- Meet the requirements for nursing facility level of care and be able to direct his/her care independently or through a responsible representative while remaining in their own home.
EARLY AND PERIODIC SCREENING, DIAGNOSIS, AND TREATMENT PROGRAM (EPSDT)
- Children from birth to age 21 - services must be prescribed by a physician. Services are medically necessary because of physical limitations due to illness or injury and require the individual to live at home. These individuals are also waiting waiver placement.
CHILDREN'S CHOICE WAIVER
- Children from birth to age 18 with developmental disabilities. This program offers supplemental support to children who live at home or leave an institution to return home.
RESIDENTIAL OPTIONS WAIVER (ROW)
- Meet the Louisiana definition for developmental disability which manifested prior to age 22, And Have an OCDD Statement of Approval AND Meet Intermediate Care Facility-Intellectual Disability (ICF-ID) Level off Care Criteria
Income and resources are also a consideration in this program.
- Persons who are age 65 or older and 21 or older who are disabled according to Medicaid standarts of SSI disability criteria.
- Income and resources are also a consideration in this program.
- Immaculate Heart of Mary is assisting with providing recipients with the maximum level of independence within the home as well as in the community.
NEW OPPORTUNITIES WAIVER (NOW)
- Persons age 3 years and older and mentally retarded (MR) or developmentally disabled (DD) which manifested prior to age 22.
- Must meet the Louisiana definition for MR/DD.
- Income and resources are also a consideration in this program.

What are Personal Care Services?
Personal care services provide non-medical hands-on assistance with activities of daily living for older adults and individuals living with disabilities. Our caregivers either assist the individual with the personal care task or provide supervision and prompting so the individual performs the task by him/herself.
